Little robust, reliable evidence supports current recommended dosing schedules, and no clinical studies assess the effectiveness and optimal dosing of intramuscular cobalamin preparations for symptom relief, other than studies assessing attained serum B 12 concentrations and sometimes haematological response
Each dose should be taken on the same day every week at any time of day

Patients with this condition or those with a family history suggestive of Lebers disease should avoid cyanocobalamin-containing products or use alternative forms of vitamin B12 such as hydroxocobalamin under careful medical supervision
Recommendation: Start b12 supplementation immediately, independent of semaglutide use
